Agentic AI failure rate hits 40 per cent: the UK ops leader checklist

Gartner expects more than 40 per cent of agentic AI projects to be cancelled by 2027

The headline figure is from a Gartner press release dated 25 June 2025, polling more than 3,400 organisations actively investing in the technology, naming three causes: escalating costs, unclear business value, and inadequate risk controls.

Anushree Verma, Senior Director Analyst at Gartner: "Most agentic AI projects right now are early-stage experiments or proof of concepts that are mostly driven by hype and are often misapplied."

Gartner also estimates that of the thousands of vendors marketing agentic features, only around 130 offer real agentic capabilities. The rest repackage chatbots, RPA, or single-step automations as agents.

Accenture's 20 April 2026 research surveyed 1,891 employees and 510 leaders across UK and Ireland. Matt Prebble, Head of Accenture UK and Ireland: "almost half of executives (46 per cent) report that AI has so far delivered little impact on profit and loss."

BDO's 2026 UK mid-market survey of 500 leaders at 10 to 300 million pound firms: 88 per cent increasing headcount; 36 per cent identified technology and AI as the primary route to growth.

UK leaders are committed to AI, but the lever has not produced the P&L lift yet. The 28 per cent succeeding cohort connected systems first.

The pattern in the failures: action layer first, insight layer never

The UK ops leader checklist before greenlighting any agentic AI deployment

Require evidence on each of the seven items below. If three or more are missing, the deployment is in the failure cohort.

1. Integration breadth is real, not promised

6. The vendor is one of the 130, not one of the thousands

Ask the vendor to demonstrate the agent making a decision your team did not pre-script. If every path traces back to a hard-coded template, you are buying RPA dressed as agency.
