AI agent governance: the UK mid-market playbook
Only 21 per cent of organisations have a mature governance model for AI agents, according to Deloitte's 2026 State of AI in the Enterprise survey of 3,235 IT and business leaders across 24 countries. Adoption is moving faster than the controls.
Three quarters of firms expect to use agents at least moderately by 2027, but most cannot yet draw the line between what an agent is allowed to do alone and what needs human approval. The fix is not slower adoption. The fix is a separation of duties: a read-only insight team that finds the work, and an action team of agents that executes it inside narrow, audited boundaries.
The 21 per cent number that should be in every UK board pack
Deloitte (the global professional services firm) surveyed 3,235 IT and business leaders across 24 countries for its 2026 State of AI in the Enterprise report, published April 2026. The headline finding: only 21 per cent of respondents say their organisations have a mature governance model in place for agentic AI.
Roughly four in five companies are deploying or about to deploy autonomous agents without the four capabilities Deloitte names as table stakes: clear boundaries on what an agent can decide alone, real-time monitoring that flags anomalies, audit trails that record every agent action, and a named human accountable for the output.
That is the gap UK boards are now expected to close. Most are not.
Adoption is moving faster than the controls, not behind them
The same Deloitte survey found 74 per cent of respondents expect their companies to use AI agents at least moderately by 2027, with 23 per cent expecting extensive use and 5 per cent expecting full integration as a core business component.
Buyers are not hesitating on demand. They are hesitating on the wiring around the demand. Deloitte's senior technology editor Andy Bayiates puts it plainly: "AI agents are scaling faster than their guardrails."
Gartner's September 2025 survey of 360 IT application leaders across North America, Europe, and Asia Pacific corroborates the gap. Only 13 per cent strongly agree their organisation has the right governance structures in place to manage AI agents. 74 per cent believe AI agents represent a new attack vector into their organisation. And only 19 per cent had high or complete trust in their vendor's ability to provide adequate hallucination protection.

The five governance failures that send agentic AI projects to the cancellation pile
Gartner (the research firm) predicts more than 40 per cent of agentic AI projects will be canceled by the end of 2027, citing escalating costs, unclear business value, and inadequate risk controls (June 2025 release, poll of 3,400 organisations actively investing in the technology). The third cause is governance, but the first two are governance dressed in commercial clothing. A vendor whose costs run away has no spending control. A vendor whose value is unclear has no measurement framework. Both are governance failures.
Across the Deloitte and Gartner data, five failure patterns repeat:
- No defined boundary between agent autonomy and human approval. The agent can act on a recommendation today, on a recommendation worth ten thousand pounds tomorrow, and on a contract change next week. No written threshold, so every decision is a coin flip.
- No real-time monitoring. The team finds out an agent took a wrong action when a customer complains, not when the action happens.
- No audit trail. When the regulator or a director asks why the agent did what it did, the answer is "we will look into it."
- No named owner. The agent is procurement's win, IT's deployment, and operations' problem. No one signs off on its weekly outputs.
- No kill switch. Pulling the agent out of production requires a vendor ticket, not a button.

Clear boundaries
For every agent in production, there is a written list of decision types it can take alone, and a list it must escalate. The list is reviewed quarterly. The list is enforced in code, not in policy documents. Without this, the 21 per cent number is theoretical.
Real-time monitoring
Every agent action is logged at the moment it happens, with the inputs that led to it. Anomalies (unusual cost, unusual scope, unusual pattern) trigger a human review within hours, not days. This is what separates agents from RPA scripts: the supervision is continuous, not scheduled.
Audit trails
Every agent action can be traced to a source event, an evidence set, and a policy. The trail is queryable by date, agent, customer, and outcome. Mid-market firms in regulated sectors (financial services, healthcare, professional services) treat this as a compliance line item, not a nice-to-have.
Accountable human
One named person per agent, signed in writing, with the authority to pause the agent, change its scope, or remove it. The person sees the weekly performance report and approves changes to its boundaries. Without an accountable owner, governance dissolves into committee meetings.

The lowest-risk way to start is to refuse to deploy an action agent until a read-only insight agent has run for two weeks and produced a numbered list of what is broken. That is the order. 1. A written boundary policy per agent
The agent's decision scope is enumerated in a one-page document signed by the accountable executive. Decisions above a defined value, or outside a defined system, are escalated to a human. The boundary is enforced in the agent's code, not relied on as a guideline.
2. A real-time activity log queryable by anyone in the room
Every action the agent takes is timestamped, attributable, and visible in a dashboard the COO or CFO can open without IT support. If you cannot show last Tuesday's actions to an auditor in five minutes, you do not have a log.
3. A defined success metric the board can read in a number
"Productivity gains" is not a metric. Hours of human work avoided, tickets resolved without escalation, ARR recovered, are metrics. Pick two and report them weekly.
4. A named human owner with a signature
Gartner's April 2026 release names executive sponsorship and integration into existing workflows as the two strongest predictors of success. An agent without a named owner drifts. An agent with a CFO or COO sign-off accumulates evidence.
5. A kill switch and an incident playbook
Pausing the agent is a one-click operation, available to the named owner. The incident playbook covers three scenarios: the agent took a wrong action; the agent stopped acting; the agent's costs spiked. Walk through each playbook before go-live.
6. Vendor scrutiny that filters the 130 from the thousands
Gartner notes that of the thousands of vendors marketing agentic features, only around 130 offer real agentic capabilities. Ask the vendor to demonstrate the agent making a decision your team did not pre-script. If every path traces back to a hard-coded template, you are buying RPA labelled as agency.
